A new designated operator was hired on 21/11 and notification was not provided to the EHD within <br /> 30 days of the change. Any changes shall be reported to the EHD with 30 days. Submit notification to <br /> the EHD identifying all the designated operators for this facility too the EHD within 30 days. A copy was <br /> provided during inspection. <br /> 305. The Bravo box float and chain at dispenser 3/4 (diesel) failed to stop the flow of product at the <br /> dispenser when tested. All monitoring equipment shall be maintained to activate an audible and visual <br /> alarm or stop the flow of product at the dispenser when it detects a leak. The service technician <br /> adjusted the chain and verified functionality. <br /> 311. The 87 and 91 line leak detector failed to detect a leak when tested. All line leak detectors shall be <br /> capable of detecting a 3-gallon per hour leak at 10 psi. The service technician adjusted the leak <br /> detector, retested it, and it passed. <br /> Notes <br /> Current certification of financial responsibility was obtained during inspection. <br /> Secondary containment testing was done last week. <br /> ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE. <br /> THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT EHD'S CURRENT HOURLY RATE. <br /> Inspector.
