Product 91 spill container's secondary containment is not tight. The 91 spill container failed the one <br /> hour hydrostatic test. Correct by repairing the 91 spill containment into proper functioning order. Contact <br /> SJC EHD if a permit is needed for repair. A re-inspection of the testing of the 91 spill containment is <br /> required and will be charged at the currently hourly rate. Submit a 48 hour test notification of the re- <br /> testing to SJC EHD. <br /> #23c. Designated operator (D.O.) failed to document correct test dates on the monthly visual inspection <br /> report. The secondary containment testing (SB-989) was conducted 05-11-09. On the D.O. monthly <br /> report for May — August 2009, the secondary containment test date was written as 03-14-2006. D.O. <br /> Correct by ensuring all required test dates are correctly written in the D.O. monthly reports. <br /> Submit to SJC EHD by 06-28-10: <br /> 1. one copy of the completed `Return to Compliance Certification' <br /> 2. a corrective action statement for the above violations <br /> Notes: <br /> 1. Received copy of the Certification of Financial Responsibility and letter from Chief Financial <br /> Officer <br /> 2. Received copy of Secondary containment test results from 05-11-09. UST system all passed <br /> except Dispenser 3-4 but was re-tested 06-1-09 and passed. <br /> 3. A Monitoring System Certification (MSC) test was conducted 04-09-2008. The following MSC test <br /> was conducted last year 05-08-2009. Facility converting from Quick Stop facility to 76 facility. <br /> Gas pumps under construction caused delay in MSC testing. Ensure future MSC are performed <br /> in a timely manner, due every 12 months. <br /> 4. Secondary containment testing (SB 989) was recently performed 05-11-2009. The previous <br /> secondary containment testing was performed 03-14-2006. Facility converting from Quick Stop <br /> facility to 76 facility. Gas pumps under construction caused delayed in SB-989 testing. Ensure <br /> future secondary containment tests are performed in a timely manner, due every 36 months.
