PGM Tire &Axle, Inc. — Inspection date 3/24/15 <br /> San Joaquin County Environmental Health Department and CalRecycle completed a joint unannounced <br /> inspection of PGM Tire &Axle, Inc. (PGM) located at 19414 N Highway 99, Acampo. <br /> PGM is in violation of 14 CCR, Section 17351(c)—Failure to maintain one pike pole or comparable pole at <br /> least 10 feet in length to separate burning from non-burning tires. <br /> PGM is in violation of 14 CCR 17351(f)-Waste tires were stored next to flammables such as wood <br /> pallets and vegetation. Waste tires must be stored no closer than 40 ft. from flammables or the facility <br /> may obtain approval from the local fire authority that determines a different requirement is adequate. <br /> Alternative requirements approved by the local fire authority shall be subject to CalRecycle's <br /> concurrence. <br /> PGM is on a well system for its water supply. Inspectors did not confirm if the water supply was capable <br /> of delivering at least 1,000 gallons per minute for a duration of at least three hours nor whether any part <br /> of the storage yard could be reached by using not more than 500 feet of hose. <br /> Manifest forms were reviewed and found to be in order. <br /> PGM currently picks up axles and tires from modular home sites and delivers the axles and tires to <br /> PGM's Acampo location. PGM uses TPID 1107176 as both the pickup and delivery TPID on <br /> comprehensive trip logs(CTLs). CalRecycle decided to create a specific TPID number for PGM to use <br /> when picking up axles and tires from modular home locations. On these CTLs,the pickup location will <br /> show the newly created TPID number, 1819254-01,and the address and phone number of the location <br /> from where the axles and tires are picked up. The operator is advised to file CTL forms for each TPID <br /> number separately. <br />
