He decided that it would provide clarity to create a TPID number that your <br /> facility will use on comprehensive trip logs for tire pickups of modular home tires. On these comprehensive trip logs, <br /> your driver will identify the address and phone number of the site from where the tires are picked up. The TPID number <br /> is 1819254-01. You should receive this TPID certificate within the week along with the certificate for TPID 1107176 <br /> showing that PGM is a permitted facility. As CTL forms are generated, please file the CTL forms for each TPID number <br /> separately.
