March 8, 2007 <br /> NOA Project Number: E07028B <br /> may have occurred as a result of leakage from the former underground storage tank. <br /> Approximately six samples will be collected from the vicinity of the adjacent former <br /> underground storage tank at locations to be determined by on-site personnel. These samples <br /> will be collected at depths up to six feet below ground surface (bgs). <br /> 1.2 Access Agreement <br /> NOA is performing the work for the property owner. No access agreement is required. The <br /> tenant will be notified by the owner prior to initiation of the field work. <br /> 2.0 HEALTH AND SAFETY PLAN <br /> We anticipate that the health hazards that will be encountered at the Site will be minimal. <br /> Based on our understanding of the past disposal practices at the Site, we do not expect to <br /> encounter any liquid or solid waste materials nor any liquid or solid hazardous waste <br /> materials at the proposed drilling locations. However, if waste materials are encountered, <br /> we anticipate that the primary mode of contaminant exposure will be through the skin as a <br /> result of handling equipment, soil, and water. Therefore, we have prepared the following <br /> Health and Safety Plan: <br /> 1) Level D Protection. We propose that all field activities be conducted <br /> consistent with Level D protection requirements as defined by the California <br /> Occupational Safety and Health Administration (OSHA). The minimal required <br /> Level D protection equipment includes hard hat, safety glasses, steel-toed <br /> boots, earplugs, disposable rubber gloves, and appropriate work clothing. <br /> 2) Emergency Treatment. In the event of an emergency, a member of the team <br /> will that 911 and follow the instructions of the operator. In the event that a <br /> non-emergency injury occurs to any or all of the team, they will be transported <br /> to the nearest hospital that provides emergency room services (Lodi Memorial <br /> Hospital, 975 South Fairmont Avenue, Lodi, 209-334-3411). <br /> 3.0 SOIL SAMPLING PROCEDURES <br /> 3.1 Drilling Details <br /> All drilling activities will be performed by NOA. <br /> 2 "'.44 <br />
