f. Site specific hazards and/or safety practices to be observed: Avoid dust clouds <br /> and avoid direct contact with soil. <br /> 7. Engineering Controls on site: <br /> a. Traffic cones and warning tapes: set up to isolate work areas from public. <br /> b. Emergency warning devices and shut-off switches: insure driller, helper, and <br /> geologist are aware of the locations and operation of the emergency shut-off <br /> switches. <br /> c: Site specific controls and/or warning devices: to be determined. <br /> 8. Medical surveillance requirements: not applicable to this project. <br /> 9. Decontamination Procedures: <br /> a. Equipment and tools which may have been in contact with petroleum product <br /> and/or hazardous materials should be decontaminated with on site wash down. <br /> Rinsate from decontamination should be containerized in barrels and stored on <br /> site. <br /> b. Personal protective equipment will be decontaminated with detergent <br /> wash/clean water rinse (if required). Any clothing which has been saturated with <br /> potentially contaminated fluid will be disposed of in rinsate barrels. <br /> 10. Emergency Response Plan: <br /> a. Emergency evacuation signal and evacuation route: Tobe determined on <br /> site: <br /> b. Nearest accessible telephone: Project drill-rig <br /> c. Nearest accessible first aid kit: Project truck <br /> d. Nearest portable fire extinguisher: Project truck <br /> e. Immediately notify authorities (call 911 if necessary) of any accident resulting <br /> in injury to personnel. Notify site supervisor and office as soon as possible. <br /> f Immediately notify site supervisor or geologist of any condition, event or <br /> circumstance which you believe to be hazardous or unhealthy. <br /> 11. Confined Space Entry: Not applicable to this project. <br /> 12. Spill Containment Procedures: Any spill of potentially contaminated fluid will be <br /> contained with temporary earthen dikes. The spill will be cleaned up by soaking it with <br /> soil and then putting the soil into storage drums for disposal. <br /> 13. Questions: (a time for attendees to ask questions of the site safety officer to ensure <br /> understanding of the items covered. <br /> 14. Personnel in attendance shall review Site Safety Plan including route to nearest <br />
