REVISED CONDITIONS OF APPROVAL (JULY 5, 1991) <br /> JOHAN & NELLIE BARTELINK <br /> MS-89-74 <br /> Unless otherwise specified, all conditions of approval shall be <br /> complied with prior to the approval of the Parcel Map. <br /> 1. DEPARTMENT OF PUBLIC WORKS <br /> a. A Parcel Map is required. <br /> b• Irrevocable Offers of Dedication to <br /> result in 25-foot wide roadways is are required on Dodds Road, <br /> Van Allen Road, and Robinson Road along the frontage of the <br /> Remainder, with 3 20 ' x 4 20 ' road returns (snipes) at the <br /> corners. <br /> C• A 5 feet wide readway dDedication to result in a 25 foot <br /> right-of-way is required on Van Allen Road along the frontage <br /> of Parcel #1. <br /> d. The frontage improvements for Van Allen Road shall be <br /> constructed in conformance with the standards for one-half of <br /> a 50 foot right-of-way rural street. The construction of the <br /> frontage improvements may be deferred at the option of the <br /> Department of Public Works, in which event the owner .shall <br /> execute a Deferred Frontage Improvement Agreement prior to <br /> approval of the Parcel Map. (A $250. 00 fee is required for <br /> processing per Ord. No. 3337 in addition to a copy of the <br /> Grant Deed and an engineer's estimate with a drawing of the <br /> required frontage improvements on a form to be supplied by the <br /> Department of Public Works) . <br /> 2 . PUBLIC HEALTH SERVICES <br /> a. Construction of an individual domestic water well under a well <br /> permit and inspection, at the time of development, by the <br /> Public Health Services. <br /> b. Construct an individual sewage disposal system under a permit <br /> and inspection, at the time of development by the Public <br /> Health Services prior to occupancy. <br />
