HEALTH AND SAFETY PROGRAM <br /> Geo-Phase Environmental Inc. proposes the following health and safety precautions, which are <br /> appropriate working on sites where workers may come in contact with hazardous waste or <br /> hazardous materials. <br /> I. Emergency Contacts <br /> The following is a list of emergency numbers and health agencies to be contacted in the event of <br /> an emergency: <br /> Local County Environmental Agency: (209) 468-3420 <br /> San Joaquin County-Public Health Services-Environmental <br /> Health Division <br /> Company/Site Health and Safety Officer: (209) 569-0293 <br /> Steve Lankford-Geo-Phase Environmental Inc. <br /> Local Fire Department: 911 <br /> Manteca Fire Department <br /> Nearby Hospital: (209) 944-5550 <br /> Dameron Hospital 525 Acacia <br /> The route to the hospital is as follows: <br /> 1. Proceed north on Hunter for 10 blocks. <br /> 2. Turn left(west)on Acacia and proceed 7 blocks <br /> 3. The hospital is on the right at the corner of Acacia &Lincoln <br /> II. Workers Health and Safety Protocols <br /> Work Space Safety <br /> The perimeter of the work space should, if possible, be 20 feet from the drilling activity. All work <br /> performed in publicly accessible areas will have a working perimeter to be marked by brightly <br /> colored delineators. No unauthorized personnel will be allowed access into the work space. The <br /> project supervisor will coordinate all field activities and maintain the safest possible working <br /> environment at all times. If an emergency occurs, the supervisor will immediately contact the <br /> proper authorities (listed above) <br /> Skin and Head Protection <br /> Disposable suits, boots, and gloves will be worn when the working conditions require such <br /> precautions. Disposable Nitrile gloves will be worn at all times during soil and groundwater <br /> sampling. Used disposable clothing will be properly discarded at the end of each working day. <br /> Respiratory Protection <br /> Half face respirators will be available for use should the situation warrant their use. If these are <br /> not adequate protection for conditions experienced, the site will be closed until appropriate <br /> respiratory protection is obtained.
