A site map was not completed or submitted as part of the business plan. A site map shall contain a north orientation, loading areas, internal roads, adjacent streets, storm and sewer drains, access and exit points, emergency shutoffs, evacuation staging areas, hazardous material handling and storage areas, and emergency response equipment. If a site map element is not applicable for your facility then list it on the map and label as "NA". Immediately log into the California Environmental Reporting System (CERS)at http://cers.calepa.ca.gov/, upload the correct or updated information, and submit to the EHD for approval. This was corrected on site. This is a minor violation. 12 HSC 25505(c)Failed to have the business plan readily available to facility or EHD personnel. Facility could not provide the Emergency Response Plan and the Chemical inventory upon request. The emergency response plans and procedures,the inventory of information required by this article, and the site map required by this section shall be readily available to personnel of the business or the unified program facility with responsibilities for emergency response or training pursuant to this section. Immediately make arrangements to have the business plan readily available for review. This was corrected on site. This is a minor violation. 15 HSC 25510(a)Failed to report actual or threatened release to the EHD and/or CaIOES Warning Center. A release of diesel of an unknown amount occurred within the last couple of months and was not reported to the EHD and CaIOES. A release or threatened release of a hazardous material shall be verbally reported immediately to the EHD (209)468-3420 and CaIOES (800)852-7550. The handler or an employee, authorized representative, agent, or designee of a handler, shall, upon discovery, immediately report any release or threatened release of a hazardous material, or an actual release of a hazardous substance, as defined in Section 374.8 of the Penal Code, to the UPA, and to the office, in accordance with the regulations adopted pursuant to this section.The handler or an employee, authorized representative, agent, or designee of the handler shall provide all state, city, or county fire or public health or safety personnel and emergency response personnel with access to the handler's facilities. Submit a written statement to EHD assuring that any future releases or threatened releases of a hazardous material will be reported immediately to the EHD and CaIOES. This is a Class II violation.
