Each facility that generates waste tires has a unique Tire Program Identification Number(TPID). It is my understanding <br /> that your main TPID#s are <br /> a) 1047383-01 assigned to 3183 S. Golden Gate Blvd., Fresno; and <br /> b) 1344985-01 assigned to 3131 E. Fremont St., Stockton. <br /> 2.)Vehicles from your company that are Registered Waste Tire Haulers are registered under the Fresno address and must <br /> list the Fresno TPID#under the hauler information on the Comprehensive Trip Log (CTL),which is on the left-hand side of <br /> the CTL Manifest receipt. (For clarification, I will refer to the vehicles registered as Used and Waste Tire Haulers as <br /> "Registered"in this email.) <br /> 3.)There are three CTL Manifest Receipts per form,which are supplied by the California Integrated Waste Management <br /> Board (CIWMB) at no cost. Contact information (to order more forms)for the CIWMB is on the top of the CTL Manifest Forms <br /> and on the CTL Field Guidance sheet that was distributed at the time of the inspection. <br /> 4.) Each leg of every trip that Used or Waste Tires are transported by a Registered vehicle MUST be manifested as a Pick- <br /> up or a Delivery on the CTL Manifest Form, reflecting the unique facility that Used or Waste tires are hauled from on the <br /> right-hand side of the CTL form.A completed CTL Manifest Form must accompany each Used and Waste Tire transport in <br /> the Registered vehicle, and a copy of the CTL receipt must be retained at the facility from which tires are transported to or <br /> from for 3 (three)years.The yellow copy(or photo copy) MUST be kept at the facility that the vehicles are registered to for <br /> three years(in your case,the Fresno location), and the hard copy(3rd part of the triplicate CTL Manifest Form) MUST be <br /> mailed to the CIWMB within 90 days of each transport.The mailed form is self-addressed and no postage is required. <br /> 4.)The Liberty Packing site located at 12045 S. Ingomar Grade Road, Los Banos, currently has no TPID#. It is my <br /> understanding that this site is operated by a customer and is not owned by your company.The Los Banos site will require a <br /> TPID#if Used or Waste Tires are transported to or from the site either by a Registered vehicle or if Used or Waste tires are <br /> transported in quantities of ten or more. New tires,or tires that have been restored to new contition, do not need to be <br /> manifested.
