This facility's EPA ID number is inactive. A hazardous waste generator shall not treat, store, dispose <br /> of, transport or offer for transportation, hazardous waste without an active EPA ID number. Immediately <br /> contact DTSC and reactivate your EPA ID number and submit evidence to the EHD by 04/04/11. A form <br /> has been provided. <br /> 45. Contingency plan is not updated. Facility's Emergency Response Plan e <br /> missing phone number for SJC EHD. Ensure all emergency response <br /> telephone numbers are current. In addition, SJC OES (Office of Emergency Services) is incorrectly listed <br /> as the CUPA but they are the PA (participating agency). SJC EHD (Environmental Health Department) <br /> is the CUPA for this region. Correct by updating the ERP to reflect correct phone numbers for the local <br /> emergency response agencies. <br /> Submit to SJC EHD within 30 days (04/04/11): <br /> 1. one copy of the complete "Return to Compliance Certification" form <br /> 2. a written corrective action statement for the above violations <br /> Notes: - In 2009, facility generated approx. 2.1 tons. In 2010, generated approx. 4.9 tons of haz. waste. <br /> - Facility has a storm water retention pond located at south end of property. <br /> - Any spillage that occurs in the bleach filter and caustic storage area are recycled into the production <br /> and does not enter into the storm water drain. <br /> - Overfill prevention on tanks via electronic radar readings and float switch, tested every 6 months. <br /> - Laboratory for quality control utilizes kitty litter for chemical spillage on the concrete floor. Waste is <br /> stored in a hazardous waste container. Sink plumbedtorecycle waste back to production. <br /> - Chlorine is transported via rail car as pressurize liquid form. Chlorine leaks as gas. Along the fence <br /> line and in the Powell Unit are gas detection meter located slightly closer to floor level. Chlorine gas <br /> heavier than air. At 3 ppm, the gas meter shuts off flow of chlorine automatically. IDLH is 10 ppm. <br /> - Universal waste and hazardous waste is located in the Warehouse. <br /> - Maintenance shop for repairs to the plant, piping, equipment. <br /> - Outside contractor, PacLease Truck aka PAPE (located in French Camp) performs vehicle <br /> maintenance. No storage of vehicle maintenance waste stored on-site - waste taken with contractor. <br /> - Silica and cellulose are used for filtering material of its metal contents (Fe, Ni). Levels of metals are <br /> below regulatory levels. <br /> - Provide free training schedule. Recommended courses: Hazardous & Universal Waste Management. <br /> ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE($122). <br /> THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT EHD'S CURRENT HOURLY RATE. <br /> EHD Inspect Received By: Title: <br /> EHD 23-02-003 Rev 08/10/10 CONTINUATION FORM <br />
