Contingency plan <br />shall describe action facility personnel shall take in response to fire, explosions, or an unplanned or non - <br />sudden release of hazardous waste. If the owner/ operator already prepared an Spill Prevention Control <br />and Countermeasure Plan (SPCC), the owner/ operator need only to amend the plan to incorporate the <br />hazardous waste management provisions. Contingency Plan must list arrangement agreed by local <br />police, fire, hospitals, clean up contractors, State (State OES) and local emergency response teams <br />(SJC EHD & OES) to coordinate emergency services. Plan will include name, address, and phone <br />numbers of emergency coordinator(s), list of all emergency equipment (fire extinguishers, spill control <br />(rags, absorbents, mops), decontamination equipment. It must include location, physical description of <br />each item on lists & brief outline of capabilities. Plan needs evacuation plan. Submit statement how this <br />will be completed. <br />79, 83. Several fluorescent tubes not placed into closed container and not identified located in the car <br />wash building. Per maintenance employee, they were changed last week from the cooler room. Failed to <br />store Universal waste lamps in a structurally sound container in a manner that prevents breakage and <br />release of mercury and other hazardous constituents to the environment and have it properly labeled. <br />Container needs to be properly identifiable and labeled as "Universal Waste -fluorescent tubes". <br />Tubes were taped together and properly labeled. Immediately place the tubes in a structurally sound <br />container. Universal waste can remain onsite for up to one year of date of waste accumulation. Ensure <br />to retain copy of disposal receipt onsite for a minimum of 3 years. <br />Submit to SJC EHD within 30 days (04/28/11): <br />1. one copy of the completed "Return to Compliance Certification" form <br />2. a written corrective action statement for the above violations <br />Notes: <br />- In 2010, facility generated approximately 7615 gallons (30.46 tons) of used oil. <br />Average 2x monthly pickups - 330 gal each pickup. <br />- In 2010, facility generated 430 gallons and 450 pounds (1.95 tons) of oily water (car wash, oily liquids) <br />and oily solids calculated from uniform hazardous waste manifest and from non -hazardous waste <br />manifest. - Hazardous waste permit fees for 2012 year will increase accordingly. <br />ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE ($122). <br />THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT EHD'S CURRENT HOURLY RATE. <br />EHD I or: <br />Received By: �^"-'� <br />Title: <br />EHD 23-02003 Rev 10/70 CONTINUATION FORM <br />
