The permit has <br /> been approved with the following conditions: <br /> 1. Contact the local fire department and the San Joaquin Valley Air Pollution Control District for <br /> any applicable permits and requirements. <br /> 2. Any changes in information and scope of work must be submitted to EHD for approval prior to <br /> commencing work. All new addenda supersede the previous ones. <br /> 3. Contact Zuna Barker at (209) 953-7311 to arrange the following appointments (a 48-hour <br /> notice is required for inspection appointments). Please be aware that inspection for emergency <br /> work must be scheduled within five days of permit approval. <br /> • 24-hour hydrostatic test (or equivalent) on UDC 1/2, 9/10 and 11/12. Submit test <br /> results within 30 days of the test being performed. <br /> • Verification of the dispenser footprints contained by the under dispenser <br /> containment sumps (UDCs). <br /> • Visual inspection of the piping within the dispensers. <br /> • Secondary containment testing on UDC 1/2, 9/10, and 11/12 is required six <br /> month after installation of penetration fittings. Provide 48 hour notification to the <br /> EHD when this test is scheduled. <br /> Please be aware that the facility cannot operate the UST system prior to the EHD <br /> inspection and approval. <br /> 4. If concrete will be broken near the tank, a secondary containment test must be <br /> performed on the tank once concrete is poured in accordance with the manufacturer's <br /> guidelines. <br /> 5. Any individual performing the work of a service technician must meet the following <br /> qualifications: <br /> a. Possess or be employed by a person who possesses one of several permissible valid, <br /> current contractor's licenses issued by the Contractors State License Board (may be issued <br /> to the installation company), or a Tank Tester's license issued to the individual by the State <br /> Water Board, and <br /> 1868 E.
