San Joaquin County APR 15 2016 <br /> Environmental Health Department <br /> 1868 East Hazelton Avenue, Stockton; California 95205-6232 <br /> Telephone:(209)468-3420 Fax:(209)468-3433 Web:www.sigov.org/ehd <br /> Underground Storage Tank Program Inspection Report <br /> Facility Name: Facility Address: Date: <br /> WEST VALLEY AUTO SERVICE LLC 2615 W GRANT LINE RD TRACY March 14, 2016 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS ll,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> 207 CCR 2715 Designated operator did not inspect all required items and/or the inspection reports not completed. <br /> There was no designated operator monthly report for March 2015 and the designated operator did not include the <br /> alarm history in the April 1, 2015 designated operator report. <br /> The designated operator documented the alarms from the attached alarm history on the February 24, 2016 and June <br /> 3, 2015, designated operator monthly inspection report but failed to check that they were responded to appropriately. <br /> The alarms included: L9 vent sump fuel alarm on February 18, 2016; L9 vent sump alarm, L12 dispenser 7/8 alarm, <br /> Q1, Q2 and 03 line leak detector, and PLLD shutdown alarms on May 26, 2015, and Q2 Premium line leak detector <br /> PLLD shutdown alarm and gross line fail alarm on May 14, 2015. The designated operator noted"Call for service."but <br /> the site has no repair or follow up records. During the monthly inspection,the designated operator shall review the <br /> alarm history for the previous month, check that each alarm was documented and responded to appropriately, and <br /> attach a copy of the alarm history with documentation taken in response to any alarms to the monthly report. Ensure <br /> that designated operators performing monthly inspections at this facility are including all of the required information on <br /> the reports. <br /> The designated operator failed to check and note the correct test dates for the secondary containment testing on the <br /> April 2015 through February 2016 designated operator monthly inspection report. The designated operator employed <br /> from April 2015 through October 2015 indicated that the secondary containment testing was completed on 3/27/12 <br /> and the current designated operator indicated that the secondary containment testing was completed on 3/27/2015; <br /> the correct date for the secondary containment testing is 3/30/2015. The current designated operator corrected the <br /> dates for the November 2015 through February 2016 designated operator monthly inspection reports. During the <br /> monthly inspection, the designated operator shall check that all required testing and maintenance for the UST system <br /> have been completed and document the dates they were done. Ensure that designated operators performing monthly <br /> inspections at this facility are including all of the required information on the reports. <br /> This is a repeat violation, Class II. <br /> 320 HSC 25290.1(c3), 25290.2(c3)Secondary containment not constructed to prevent water intrusion. <br /> Water was found in the vent sump, diesel fill sump, diesel submersible turbine pump(STP)sump, 87-product STP <br /> sump, 91-product STP sump, dispenser 314 under disperser container(UDC)and disperser 7/8 UDC. Secondary <br /> containment shall be constructed to prevent any water intrusion into the system by precipitation, infiltration,or surface <br /> runoff. The service technician removed approximately one gallon of liquid from the vent sump, approximately two <br /> quarts of liquid from the diesel fill sump, approximately one quart of liquid from the diesel STP sump, approximately <br /> one quart of liquid from 87-product and 91-product STP sumps, oily debris from dispenser 3/4 and approximately two <br /> cups of liquid (possibly product)from dispenser 7/8 and returned the sensor to its proper location. Immediately <br /> contact a properly licensed, trained, and certified contractor to address the possible product found in disperser 7/8 <br /> UDC and water intrusion found in the vent, diesel fill, diesel STP, 87-product STP, 91-product STP sumps and <br /> dispenser 3/4 UDC under permit and inspection of the EHD. <br /> This is a repeat violation, Class II. <br /> Page 5of6 <br />
