Grant Line RD <br /> Tracy CA 95304 <br /> The San Joaquin County Environmental Health Department (EHD) has completed the review of <br /> the application for a permit to install Icon electrical conduit penetrations in the vent sump and <br /> UDC 7/8 on the underground storage tank (UST) system at the above noted address. The <br /> permit has been approved with the following conditions: <br /> 1. Contact the local fire department and the San Joaquin Valley Air Pollution Control <br /> District for any applicable permits and requirements. <br /> 2. Any changes in information and scope of work must be submitted to EHD for approval <br /> prior to commencing work. All new addenda supersede the previous ones. <br /> 3. Contact Michelle Henry at (209) 468-3454 to arrange the following inspection <br /> appointments (a 48-hour notice is required for inspection appointments). Please be <br /> aware that inspections for emergency work must be scheduled within five days of permit <br /> approval. <br /> a. Sensor functionality: Functionality of the monitoring system must be verified to <br /> ensure that monitoring has been restored to meet the requirements of Title 23 <br /> California Code of Regulations and the Health & Safety Code. <br /> b. 24-hour hydrostatic test (or equivalent) on vent sump and UDC 7/8. <br /> 4. Secondary containment testing for the vent sump and UDC 7/8 is required six months <br /> after installation. Provide 48 hours notification to the EHD when this test is scheduled. <br /> 5. Qualified service technicians must possess or work under the direct and personal <br /> supervision of an individual physically present at the work site who possesses all of the <br /> following: <br /> a. One of several permissible valid, current contractor's licenses issued by the <br /> Contractors State License Board (may be issued to the installation company), or a <br /> Tank Tester's license issued to the individual by the State Water Board, and <br />
