The designated operator failed to attach the Q1:Regular, Q2:Premium and Q3:Diesel alarm <br /> history for all twelve months of the designated operator monthly inspection report and failed document the dates for <br /> L9: Vent Sump and L5: Dispenser 3/4 that occurred on December 12, 2014 and February 10, 2015 in the designated <br /> operator monthly report. During the monthly inspection, the designated operator shall review the alarm history for the <br /> previous month, check that each alarm was documented and responded to appropriately, and attach a copy of the <br /> alarm history with documentation taken in response to any alarms to the monthly report. Ensure that designated <br /> operators performing monthly inspections at this facility are including all of the required information on the reports. <br /> This is a repeat violation, Class II. <br /> ,i <br /> 209 CCR 2715(f) Designated operator employee training not performed or log not kept. <br /> The designated operator employee training log did not have the hiring dates of all employees. A list of facility <br /> employees who have been trained by the designated operator(s), shall be maintained on-site and shall include the <br /> dates of training for all facility employees, and hiring dates for all facility employees hired on or after July 1, 2005. <br /> This is a minor violation. <br /> 315 HSC 25290.2(c)(1) Primary containment not tight (Jul 2003-Jun2004). <br /> Observed 1/2 cup of fuel in the 87 STP sump, indicating a leak in the primary piping. Service technician removed the <br /> fuel and left on site the hazardous waste drum. All primary containment for the UST system must be tight. <br /> Immediately have a properly licensed, trained, and certified contractor repair or replace the failed component. A permit <br /> may be required from the EHD. Provide a statement to EHD within 30 days on how the leak was corrected. <br /> This is a minor violation. <br /> 320 HSC 25290.1(c3), 25290.1(c3) Secondary containment not constructed to prevent water intrusion. <br /> Water was found in the Diesel STP (approximately 2 cups), Diesel Fill(approximately 1 cup) and Vent sumps <br /> (approximately 3 cups). Secondary containment shall be constructed to prevent any water intrusion into the system <br /> by precipitation, infiltration, or surface runoff. The service technician removed the liquid and returned the sensor to its <br /> proper location at the time of inspection. Immediately contact a properly licensed, trained, and certified contractor to <br /> address the water intrusion into the Diesel STP, Diesel Fill, and Vent sumps under permit and inspection of the EHD. <br /> This is a repeat violation, Class II. <br /> Overall Inspection Comments: <br /> "*Inspection conducted on March 25, 2015.Checklist was left on site at time of inspection.
