SAN ��JOAQU I N Environmental Health Department <br /> 0U -i AMENDED <br /> 17 .N <br /> Underground Storage Tank Program Inspection Report <br /> Facility Name: Facility Address: Date: <br /> WEST VALLEY AUTO SERVICE LLC 2615 W GRANT LINE RD, TRACY March 05, 2018 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I, CLASS II,or MINOR-Notice to Comply) <br /> Item # Remarks <br /> 112 HSC 25284.2 Failed to test spill containers annually. <br /> Annual spill container testing was last performed on Mach 14, 2016 and was completed on June 23, 2017, about <br /> four months past due. This test is required once every 12 months. Take all necessary precautions to ensure <br /> testing is performed in a timely manner. <br /> This is a Class II violation. <br /> 315 HSC 25291(e) Water in secondary containment not removed, analyzed, and properly disposed of(pre-Jul 2003). <br /> Liquid was observed in the 87 and 91 submersible turbine pump (STP) sumps and the under dispenser containment <br /> (UDC) sumps at dispenser 3/4 and 7/8. If water could enter into the secondary containment by precipitation or <br /> infiltration, it must be removed and disposed of properly. The service technician removed about 6 cups of liquid <br /> from the 87 STP sump and about 4 cups of liquid from the 91 STP sump. The service technician removed about 3 <br /> cups of liquid from the UDC sump at dispenser 3/4 and about 2 cups of liquid from the UDC sump at dispenser 7/8. <br /> Ensure that all sumps and annular spaces are maintained free of liquid. <br /> This was corrected on site. <br /> This is a repeat violation, Class II. <br /> Overall Inspection Comments: <br /> An inspection report was provided to the facility operator on the day of inspection. The report was amended by <br /> the EHD. The amended report replaces the initial report. <br /> Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br /> documenting the corrective actions that have been or will be taken for each violation, and any supporting <br /> paperwork, within 30 days of receiving the amended inspection report. <br /> Violation for liquid in secondary containment was corrected on site. No open violations and a return to <br /> compliance is not needed. <br /> Documents reviewed by regulator: Employee training log, monitoring system certification reports (2015, 2016, <br /> 2018), secondary containment report (2015), operating permit, financial documents and letter from chief <br /> financial officer, Designated operator monthly inspection reports, maintenance records, monitoring plans, <br /> response plan, alarm history and setup. <br /> Document received by operator: Inspection Report. <br /> Note: A routine secondary containment testing was witnessed during time of inspection. Testing was not <br /> completed while inspector on site. The 87, 91, and Diesel submersible turbine pump (STP) sumps passed the <br /> 5psi one hour vacuum testing. The hydrostatic testing on the 91 STP sump passed. The test boots on the 87 <br /> and the Diesel STP sumps failed and needed replacement. Work is being conducted. The hydrostatic testing did <br /> not pass.
