San Joaquin County <br /> Environmental Health Department <br /> 1868 East Hazelton Avenue, Stockton, California 95205-6232 <br /> Telep one:(209)468-3420 Fax:(209)468-3433 Web:www.s4gov.org/ehd <br /> Large Quantity Hazardous Waste Generator Inspection Report <br /> Facility Name: Facility Address: Date: <br /> Global Materials Management, L C 1 1640 N Broadway Ave, Stockton August 26, 2016 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> 301 CCR 66265.31 Facility not maintained to minimize the release of a hazardous waste. <br /> Inside.- <br /> -the <br /> nside:-the used oil filter crusher appeared to be spilling or leaking used oil onto the floor <br /> -one thirty gallon "PENNZ IL"and multiple other oily drums, containing mixture of absorbents, oil equipment, and oil <br /> were seen on or near oily absorbent on the ground <br /> -multiple double stacked rows of plastic totes seen with free flowing oil on the ground (oil identified by Mr. Kandathil) <br /> -west of the oil filter crusher, used oil and oily debris, including filters, seen on the floor <br /> Facilities shall be maintai ied and operated to minimize the possibility of a fire, explosion, or release of hazardous <br /> waste to air, soil, or surface water which could threaten human health or the environment. Immediately begin the <br /> practice of minimizing the release of hazardous waste, clean the impacted areas, and manage according to Title 22 <br /> hazardous waste regulatic ns. Submit a statement and supporting documentation explaining how these wastes were <br /> managed. <br /> This is a Class II violation. <br /> 302 CCR 66265.32 Failed to maintain required emergency equipment. <br /> Internal communication or alarm system, a device capable of calling outside emergency help, spill control equipment, <br /> and decontamination equipment were not found on site. All facilities shall be equipped with an internal <br /> communication or alarms stem, a device capable of calling outside emergency help, spill control equipment, and <br /> decontamination equipment at all times. Immediately provide internal communication or alarm system, a device <br /> capable of calling outside amergency help, spill control equipment, and decontamination equipment equipment and <br /> maintain on site. <br /> This is a Class II violation. <br /> 303 CCR 66265.33 Failed to maintain all communication or alarm systems, spill control, or decontamination equipment. <br /> Fire extinguishers at the fE cility had not been maintained. Annual maintenance was last performed June 29, 2015. All <br /> facility communications or alarm systems, fire protection equipment, spill control equipment, and decontamination <br /> equipment, where required, shall be tested and maintained as necessary to assure its proper operation in time of <br /> emergency. Immediately begin this practice. <br /> This is a Class II violation. <br /> 304 CCR 66265.35 Failed to maintain adequate aisle space for unobstructed response to emergencies. <br /> The facility was overwheIr led with 55 gallon drums and —300 gallon totes inside the building and outside. On the <br /> South side of the buildinghere were over two hundred 55-gallon drums of used oil, hazardous waste, used oil filters, <br /> and misc. It was difficult a times for inspectors to maneuver between drums. Adequate aisle space shall be <br /> maintained to allow the un bstructed movement of personnel, fire protection, spill control equipment, and <br /> decontamination equipme t to all areas of facility operation in an emergency. Immediately maintain aisle space to <br /> allow the unobstructed movement of personnel, fire protection, spill control equipment, and decontamination <br /> equipment to any area of f 3cility operation in an emergency. <br /> This is a Class II violation. <br /> Page 9of15 <br />
