11/12/99 <br />Received analytical results from GeoAnalytical. The results indicated several samples exceeded the <br />regulatory levels listed in Title 22. MK <br />11/18/99 <br />Revised Unified Hazardous Waste Inspection Report to include a new violation for disposing hazardous <br />waste on the ground. <br />Mailed revised inspection report, copy of analytical result and chain of custody and a set of photo taken <br />during inspection via certified mail. MK <br />1/13/00 <br />Received certificate of return to compliance; letter dated 1/11/00 from Peter Warren, General Manager, <br />addressing the violations, corrections and requesting EHD to rescind the violations; copy of analytical <br />result from Sequoia Analytical and chain of custody; and copy of sections of the law. MK <br />2/7/00 <br />After reviewing laws and response from Mr. Warren, I prepared a letter informing Mr. Warren that his <br />request was denied. See letter. <br />The certificate of return to compliance was signed and dated before violation # 19, storage of HW longer <br />than 90 days without permit or authorization, had been corrected. I did not mention about the perjury issue <br />on that violation since Mr. Warren claimed in his letter that the waste would be shipped offsite no later that <br />1/31/2000. <br />Mailed letter via certified mail.
