All samples will be refrigerated using <br /> wet ice at 4°C (+/-2°C) and delivered to the analytical laboratory within 48 hours of <br /> collection. All samples will be delivered to the laboratory by commercial courier or CRA <br /> personnel. All samples will be maintained at 4°C (+/-2°C)by the laboratory. <br /> The Chain of Custody record,completed at the time of sampling,will contain,but not be <br /> limited to, the sample number, date and time of sampling, and the name of the sampler. <br /> The chain of custody document will be signed, timed, and dated by the sampler when <br /> transferring the samples. <br /> Each sample cooler being shipped to the laboratory will contain a Chain of Custody <br /> form. The Chain of Custody form will consist of four copies which will be distributed to <br /> the shipper, the receiving laboratory, and two copies to CRA. The shipper will maintain <br /> one copy while the other three copies will be enclosed in a waterproof envelope within <br /> the cooler with the samples. The sample ID of each sample shipped will be recorded on <br /> the sheet. The cooler will then be sealed properly for shipment. The laboratory, upon <br /> receiving the samples, will complete the three remaining copies. The laboratory will <br /> maintain one copy for their records. One copy will be returned to CRA upon receipt of <br /> the samples by the laboratory. One copy will be returned to CRA with the data <br /> deliverables package. <br /> Upon receipt of the cooler at the laboratory, the shipping cooler will be inspected by the <br /> designated sample custodian. The condition of the cooler will be noted on the Chain of <br /> Custody record sheet by the sample custodian. If the shipping cooler seal is intact, the <br /> sample containers will be accepted for analyses. The sample custodian will document <br /> the date and time of receipt of the container, and sign the form. The sample custodian <br /> will also record the temperature of one sample (or temperature blank) from each cooler <br /> and the temperature will be noted on the Chain of Custody. <br /> If damage or discrepancies are noticed (including sample temperature exceedances), <br /> they will be recorded in the remarks column of the record sheet, dated and signed. Any <br /> damage or discrepancies will be reported to the lab supervisor who will inform the <br /> laboratory project manager and laboratory QA Office (QAO) before samples are <br /> processed. The QAO shall be notified immediately about any discrepancies noted. <br /> 6.3 SAMPLE DOCUMENTATION IN THE LABORATORY <br /> Each sample or group of samples shipped to the laboratory for analysis will be given a <br /> unique identification number.
