Lodi Ave. <br /> Lodi, CA, 95242 <br /> San Joaquin County Public Health Services, Environmental Health Division (PHS-EHD), has reviewed this <br /> site file and has discovered that there has not been a groundwater sampling event since March 14, 1996. <br /> In order to properly assess this site, quarterly groundwater monitoring is, and has been required for this <br /> site since its entry into the PHS-EHD Local Oversight Program. Your failure to provide this minimum <br /> monitoring infbnTiation has now placed your site in non-compliance and you in jeopardy of becoming <br /> ineligible to obtain a Letter of Commitment from the State Water Resources Control Board (SWRCB), <br /> Cleanup Fund. <br /> You are hereby directed to Initiate quarterly groundwater monitoring immediately,starting with <br /> this Third Quarter-1997(July,August,September)and report the results to PHS-EHD before the <br /> next quarter's sampling evert. In addition,by January 15, 1998,you are hereby directed to submit <br /> a workplan to delineate the lateral and vertical limits of the contamination in the soil and <br /> groundwater at and around your site. <br /> Failure to provide a complete Third Quarter Monitoring Report by November 7, 1997 may result in formal <br /> notification to the SWRCB that you have failed to comply with directives issued by PHS-EHD. Failure to <br /> provide the required workplan by January 15, 1998 may result in fartnal enforcement action and possible <br /> referral to the Central Valley Regional Water Quality Control Board (CVRWQCB). <br /> PHS-EHD recommends that you evaluate your current site status and make every attempt to bring your <br /> site into compliance.
