A 3.5-foot diameter, 4-inch thick concrete pad will be <br /> poured surrounding each well. <br /> Well Cap—2-inch diameter expandable, lockable pressure plug. <br /> Surface Seal—Neat cement or cement grout will be placed from the land surface to a minimum depth of 20 <br /> feet and to the top of the bentonite seal. A minimum 2-feet of granular bentonite seal will be placed <br /> from the top of the filter pack to the bottom of the neat cement surface seal. <br /> Note that, where required,the above ground completion of the monitoring wells will have a minimum of three <br /> 6-inch diameter steel bollards filled with concrete. The above surface completion program is preferred to one <br /> where the top of the monitoring well is encased in a subsurface box set at grade. The latter runs the risk of <br /> having the well polluted by surface water that enters the box and covers the top of the monitoring well. <br /> Figures 4 and 5 depict a typical monitoring well completion anticipated for this site. <br /> EQUIPMENT DECONTAMINATION PROCEDURES <br /> Decontamination will comprise a hot water pressure wash of the drilling equipment and well purging and <br /> flushing equipment with a non-phosphate detergent, followed by a clean water rinse. As a precaution against <br /> cross-contamination between boreholes, the drilling and flushing equipment will be decontaminated prior to <br /> the start of drilling or flushing of the first monitoring well and between each successive monitoring well. <br /> HEALTH AND SAFETY PLAN <br /> Personal Protective Equipment <br /> The Kellogg facility currently directs contact water(water in contact with composting materials)to the Contact <br /> Water Pond for reuse or evaporation. Risks of exposure to hazardous chemical or biological agents originating <br /> from the effluent is minimal. The practice of good personal hygiene by the crew is warranted and will further <br /> reduce the exposure risk.
