RECEIVED <br /> Return to Compliance Certification MAY 2 2 2017 <br /> April 21, 2017 Inspection Date ENVIRONMENTAL HEALTH <br /> DEPARTMENT <br /> Item# 102 <br /> Sanborn Chevrolet, Inc. has contacted our Waste Hauler, Safety Kleen and they have delivered a <br /> bucket labeled properly for the Body Shop Sanding Dust. An accumulation Start Date has been placed <br /> on the label and the hauler has been notified to include the Body Shop Sanding dust on a 90 day pick <br /> up schedule. The Sanding Dust will be swept and disposed of in the bucket. Pictures of the sanding <br /> dust bucket are attached. <br /> Item# 107 <br /> Sanborn Chevrolet, Inc. has logged on to CERS and completed the Contingency plans for Sanborn <br /> Chevrolet Service and Sanborn Chevrolet Quick lube. Both plans were accepted and are available on <br /> the CERS website. <br /> Item# 114 <br /> Manifest number 004931454SKS was missing the signed copy from the destination facility. The signed <br /> copy was provided by the inspector and this item was corrected on site. Copies are attached. <br /> Item# 118 <br /> Jason Curl—Emergency Response Coordinator and Service and Parts Director and Heather Fraser- <br /> HR/Office Manager held departmental meetings for personnel who handle hazardous waste to train <br /> each one to respond effectively to emergencies at the dealership through emergency procedures, <br /> emergency equipment and the Sanborn Chevrolet emergency systems. On site are the signed training <br /> certificates for each and every current employee, stating that they have read and understand the <br /> procedures for emergency response of a hazardous material as well as evacuation procedures,fire <br /> response and spill response. Attached are the training certificates for Jason Curl and Heather Fraser <br /> as Hazardous Waste Management personnel. Also attached is the training packet given to each and <br /> every employee. <br /> Item#501 <br /> Sanborn Chevrolet contacted Safety Kleen, our waste hauler;they came to our facility and hauled the <br /> tanks of used oil on April 27, 2017 and waste coolant on May 11, 2017 and May 18, 2017. Attached <br /> are the manifests stating the new accumulation start dates for the waste oil tank and waste coolant <br /> tank. Each tank has a proper label with the accumulation start date. Photos are also attached. <br /> Item#502 <br /> Sanborn Chevrolet ordered a tank assessment from Bagley Enterprises on April 24, 2017,at 9:13 am, <br /> waiting for engineer to contact to set up an appointment. <br />
