3. 0 FIELD PERSONNEL <br /> The field team will consist of the following persons: <br /> Project Manager: Paul Anderson <br /> Site Safety Coordinator: Brendon Wilder <br /> PSC Site Supervisor: Brendon Wilder <br /> PSC Subcontractors: Subdynamic Locating Services,Inc. <br /> Osborne Concrete Coring,Inc. <br /> TEG-Northern California <br /> PSC may require proof of the following health and safety requirements from its <br /> subcontractors: <br /> ' . Subcontractor employees must have appropriate training [i.e., either a 40-hour or <br /> 24-hour OSHA-required (29 CFR 1910.120) health and safety course for hazardous <br /> waste work,or certified equivalent training]. <br /> Personnel working at hazardous waste sites must have an annual physical (or <br /> ' physician's waiver for biennial physical) and be certified "fit for duty" and "fit for <br /> respirator use," if necessary,by a qualified physician. <br /> PSC may insist on obtaining proof of training and annual physical before site work <br /> begins. <br /> ' . Personnel must have appropriate personal protective equipment (PPE) for the <br /> specific job. All equipment and field operations must meet applicable safety <br /> standards. <br /> Subcontractor must sign Subcontractor Statement of Compliance Form for all on-site <br /> employees before commencement of site work. <br />
