The only well log contained in the Well/Receptor survey that <br /> matches the information in our small public water system file for the wells on site is for <br /> well #3. Our file information indicates the total depth of well #1 is 150 feet and the total <br /> depth of well #2 is 146 feet. The construction of drinking water well #1 is unknown and <br /> this well has been kept at the site in case of failure of one of the other wells. This well is <br /> in the MTBE groundwater contaminant plume and should not be used. Please submit a <br /> proposal for destruction of this well. <br /> The lateral and vertical extent of the MTBE groundwater contaminant plume has not <br /> been defined. A work plan to investigate the lateral and vertical extent of the <br /> groundwater contamination must be submitted to PHSIEHD. The work plan must <br /> include at least one deeper screened monitoring well in the contaminant source area. <br /> The necessity of additional deep-screened wells should be based on the information <br /> generated during installation of the deep screened monitoring well in the source area. <br /> Additional shallow screened monitoring wells are necessary to define the lateral extent <br /> of the shallow groundwater contaminant plume.
