The following is an itemized list of hazardous waste violations that have not been addressed for HOLT OF CALIFORNIA as of June 09, 2015. Open violations from May 04, 2015 inspection Violation #403 - Failed to keep hazardous waste containers closed except when adding or removing hazardous waste. Bead blast waste was observed uncontainerized on the floor of the shop. According to Mr. DuCharme, this waste w� left when the bead blast machine was moved on Friday (5/1/15) afternoon. All hazardous waste containers shall be closed at all times except when adding or removing waste. Immediately place this waste into properly closed containers and ensure all hazardous waste containers are closed when not adding or removing waste. Violation #501 - Failed to comply with new hazardous waste tank system assessment requirements. Facility does not have a current tank assessment for the 1,000 gallon used oil tank. The tank assessment on site was dated February 26, 2010, which is past the five year date. The owner or operator of a hazardous waste tank sh, obtain a written assessment reviewed and certified by an independent, qualified, professional engineer, registered in California attesting that the system has sufficient structural integrity, is acceptable for the transferring, storing, and treating of hazardous waste, and that the tanks and containment system are suitably designed. Immediately have ar assessment prepared for the used oil tank. Submit a schedule showing when this will be accomplished to the EHD. Violation #601 - Stored hazardous waste on site longer than 90 days without a permit or authorization. A 35 gallon black steel drum of aerosol can puncturing waste, labeled "paint waste", was observed in the hazardous waste storage area with an accumulation start date of 5/9/14. Facilities who generate more than 1000 kg of hazardous waste per month may store waste on site up to 90 days. Immediately contact a licensed hazardous wastE hauler to dispose of this waste under manifest and submit a copy of the manifest to the EHD. Violation #605 - Failed to completely label containers or tanks of hazardous waste. A 55 gallon black steel drum of bead blast waste was observed with no hazardous property or or accumulation start date on the label. Bead blast waste was observed uncontainerized and unlabeled on the floor of the shop. Accordin to Mr. DuCharme, this waste was left when the bead blast machine was moved on Friday (5/1/15) afternoon. The aerosol can puncturing waste was labeled as "paint waste"; the punctured aerosol cans observed next to the puncturer were for brake cleaner. All hazardous waste containers shall be marked with the following information: - the words "Hazardous Waste" - name and address of generator - hazardous properties (ignitable, corrosive, reactive, toxic) - physical state (liquid, solid) - composition (contents) - accumulation start date Immediately label these containers with the accurate and complete waste information or place the wastes in properly labeled containers, and ensure that all hazardous waste containers are marked with all the required information. Since it cannot be determined how long the drum of bead blast waste has been on site, immediately contact a licensed hazardous waste hauler to dispose of this waste under manifest and submit a copy of the manifest to the EHD. Violation #3020 - Unlisted Training violation. 25201.16(h)(8) No training records were available for the aerosol can puncturing processes. A universal waste handler may process universal waste aerosol can to remove and collect the contents of the universal waste aerosol can, if the universal waste handler ensures, through a training program utilizing the written operating procedures developed pursuant to paragraph (4), that each employee is thoroughly familiar with the procedure for sorting and processing universal was aerosol cans, and proper waste handling and emergency procedures relevant to his or her responsibilities during normal facility operations and emergencies.
