RECT JUL u 0 2015 <br />The following is an itemized list of hazardous waste violations that have not been <br />addressed for HOLT OF CALIFORNIA as of June 09, 2015. <br />Open violations from May 04, 2015 inspection <br />Violation #105 - Failed to determine if a waste is a hazardous waste or retain waste analysis on site for 3 <br />years. <br />No waste analysis was available for the soil/absorbent waste in bins. The disposal records on site indicate that the <br />soil/absorbent is being disposed of as non -hazardous waste. A generator shall make a hazardous waste determinatii <br />and keep a record of any test results, waste analyses, or other determinations made in accordance with hazardous <br />waste regulations for at least three years from the date that the waste was last disposed of. Immediately locate a <br />copy of the waste analysis conducted for the bin solid waste and the metal grindings and submit a copy to the EHD. <br />If a copy is unavailable, immediately make a hazardous waste determination and begin managing these wastes unde <br />Title 22 hazardous waste regulations. <br />Violation #115 - Failed to keep signed copy of manifests from the designated facility for three years. <br />Manifest number 013082557JJK (8/5/14) was not found on site. Manifest number 013079155JJK (5/8/14) was fount <br />without a signed off copy from the destination facility. No manifests were found on site for disposal of metal fines. <br />According to the facility response to the 2012 inspection, metal fines are being handled as hazardous waste. <br />Hazardous waste generators shall retain copies of all manifests for three years or until a copy signed off by the <br />destination facility is received. The signed off copy shall be retained and made readily available for review for three <br />years. If the generator did not receive a copy of the manifest with the handwritten signature of the owner or operator <br />the facility to which the generator's waste was submitted within 60 days of the date the waste was accepted by the <br />initial transporter, the generator shall submit a legible copy of the missing manifest, with some indication that the <br />generator has not received confirmation of delivery. This information shall be submitted to: <br />DTSC Report Repository <br />Generator Information Services Section <br />P.O. Box 806 <br />Sacramento, CA 95812-0806 <br />Immediately locate a copy of the missing manifests or prepare and submit the required information to DTSC. Submi <br />to the EHD a copy of the signed off copy of the manifests or a copy of the information submitted to DTSC. <br />Violation #116 - Failed to comply with uniform hazardous waste manifest exception requirements. <br />Manifest number 013082557JJK (8/5/14) was not found on site. Manifest number 013079155JJK (5/8/14) was fount <br />without a signed off copy form the destination facility. No manifests were found on site for disposal of metal fines. <br />According to the facility response to the 2012 inspection, metal fines are being handled as hazardous waste. <br />Hazardous waste generators shall retain copies of all manifests for three years or until a copy signed off by the <br />destination facility is received. If the generator did not receive a copy of the signed off copy of the manifest withii <br />45 days of the date that the waste was accepted by the initial transporter, the generator shall submit an Exception <br />Report that includes a legible copy of the missing manifest, and a cover letter signed by the generator explaining the <br />efforts to locate the hazardous waste and the results of those efforts. Exception reports shall be submitted to: <br />DTSC Report Repository <br />Generator Information Services Section <br />P.O. Box 806 <br />Sacramento, CA 95812-0806 <br />Immediately locate a copy of the missing manifests or prepare and submit an exception report to DTSC.
