• •u rt-q • <br /> 0 <br /> mm <br /> BEEN <br /> 1.0 INTRODUCTION <br /> This Health and Safety Plan describes the procedures that shall be followed to protect the health and <br /> safety of Bureau Veritas North America, Inc (Bureau Veritas)employees engaged in fieldwork at the <br /> Central Lodi Cell Site located at 114 North Main Street, Lodi, California. <br /> This Health and Safety Plan has been developed to comply with applicable federal, state, and local health <br /> and safety requirements including California Occupational Safety and Health Administration's (Cal-OSHA) <br /> requirements contained in Title 8 of the California Code of Regulations, Section 5192 (8 CCR 5192)and <br /> federal OSHA requirements contained in Title 29 of the Code of Federal Regulations, Section 1910.120 <br /> (29 CFR 1910.120). <br /> The observance and practice of the health and safety procedures specified in this plan, and compliance <br /> with applicable federal, state, and local regulations, pertaining to health and safety, are mandatory for all <br /> personnel and visitors. In the event of conflicting requirements,the procedures that provide the highest <br /> degree of required personal protection shall be implemented. <br /> A copy of the site health and safety plan shall be kept onsite and made available for inspection and <br /> review by employees, clients, agency personnel, and other visitors. <br /> 2.0 WORK ACTIVITIES <br /> The following is a brief description of the work activities to be performed at the site: <br /> 1. Perform pre-field activities that include preparing a Site Health and Safety Plan (SHSP). <br /> 2. Conduct daily tailgate meeting with all subcontractors. <br /> 3. Observe advancement of three truck mounted direct push borings to a total depth of 5 feet below <br /> ground surface (bgs)for the purpose of collected shallow soil samples. <br /> 4. Prepare a report that documents the pre-field and field activities and relevant observations. <br /> 3.0 ONSITE ORGANIZATION <br /> Each person at the site shall be responsible for following the guidelines outlined in this health and safety <br /> plan. The site safety officer is Erick Leif(or other person designated by Bureau Veritas). The site safety <br /> officer's duties are to: <br /> • Evaluate the site for actual and potential hazards prior to the commencement of work. <br /> • Oversee implementation of the health and safety plan. <br /> • Confirm that all personnel have proper training and protective equipment. <br /> • Conduct a "tailgate" meeting on the first day before field activities commence. <br /> • Stop work if the health or safety of workers is in question. <br /> SHSP Central Lodi <br />
