• uv9 <br /> e <br /> • The contents of the health and safety plan. <br /> • A discussion of the site-specific health and safety hazards, protective measures, and work practices. <br /> 7.0 MEDICAL SURVEILLANCE <br /> Prior to being assigned to a hazardous or a potentially hazardous activity involving exposure to toxic <br /> materials, employees must receive a baseline physical exam. The contents of the physical exam are to <br /> be determined by the employer's medical consultant. The baseline physical exam should categorize <br /> employees as fit-for-duty and able to wear respiratory protection. <br /> In addition to the baseline physical, employees must have a periodic physical exam every 12 months. All <br /> personnel working in contaminated or potentially contaminated areas at the site must have current <br /> medical monitoring (i.e., exam within 12 months). <br /> 8.0 PERSONAL AIR SAMPLING <br /> Based upon information about the site, personal air sampling will not be performed during site activities. <br /> However, if site workers or the Site Safety Officer determine that the potential for exposure to chemical <br /> substances exists at the work site prior to or during work activities, the Site Safety Officer will re-evaluate <br /> the current health and safety measures and revise them as necessary. <br /> 9.0 SITE CONTROL MEASURES <br /> The following safe work practices apply for the entire site: <br /> • Observe the "buddy system,"never enter or exit contaminated areas alone <br /> • Maintain line-of-sight or radio communication between personnel in contaminated and non- <br /> contaminated areas <br /> • No smoking, eating or drinking except in a designated "clean zone" <br /> • No horse play <br /> • No matches or lighters in contaminated areas <br /> 10.0 DECONTAMINATION PLAN <br /> Decontamination involves the orderly, controlled removal of contaminants. The following indicates the <br /> specific procedures for removal and decontamination of PPE: <br /> Level D: Remove outer garments (i.e., coveralls).
