In addition,the boring locations were cleared by a private <br /> utility locator service. <br /> • Contracted with a licensed (C-57) drilling company to complete the LSI using truck-mounted <br /> direct-push (Geoprobe) equipment. <br /> • Advanced three soil borings to five feet below ground surface (bgs) and collected discrete soil <br /> samples from the soil borings. <br /> • Submitted soil samples from each boring to a state-certified laboratory for chemical analysis. <br /> • Prepared this report summarizing the field activities and the laboratory analytical results. <br /> . E <br /> 3.0 PRE-FIELD ACTIVITIES <br /> Site access agreements were secured with the property owners prior to commencement of the fieldwork. <br /> Field activities were coordinated with the property owners in accordance with access requirements. <br /> Bureau Veritas obtained the appropriate drilling permit from the San Joaquin County Environmental <br /> Health Department(SJEHD). The SJEHD required a permit fee and on site grout inspector to be present <br /> during abandonment activities of the soil borings. Per the SJEHD permit requirements, a copy of the final <br /> LSI report is required to be provided to the SJEHD. The boring permit application and associated <br /> documents are appended. <br /> Bureau Veritas developed a SHSP for the work proposed in accordance with the requirements of the <br /> State of California General Industry Safety Order (GISO) 5192 and Title 29 of the Code of Federal <br /> Regulations, Section 1910.120_(29 CFR 1910.120). A copy of the SHSP was kept onsite during field <br /> activities. The SHSP detailed the work to be performed, safety precautions, emergency response <br /> procedures, nearest hospital information, and onsite personnel responsible for managing emergency <br /> situations. Prior to starting work, a "tailgate" safety meeting including discussion of the safety hazards <br /> and precautions relevant to the particular job was held with all personnel working on the job. Signatures <br /> of each person that attended the tailgate safety meeting were obtained and documented in the SHSP. <br /> 4.0 , FIELD ACTIVII <br /> TIES <br /> The following is a summary of the field activities that were completed during this investigation. <br /> 4.1 GEOPHYSICAL SURVEY AND UTILITY CLEARANCE <br /> On November 6, 2012, Bureau Veritas contracted with NORCAL Geophysical Consultants, Inc., located <br /> in Cotati, California, to complete a geophysical survey using ground penetrating radar(GPR) of an <br /> approximately 480 square-foot area of the subject property and to clear the three proposed boring <br /> locations. The geophysical survey did not identify anomalies of significance and/or indicative of large <br /> subsurface features such as USTs.The geophysical survey documentation is included in Appendix A. In <br /> addition, the three boring locations located within the 480 square-foot geophysical survey area were <br /> cleared of potential underground utilities. <br /> 2 <br />
