Church St. Lodi, CA 95241 Program: HW
SUMMARY OF VIOLATIONS
CLASS I, CLASS II, or MINOR-Notice to Comply)
605. A container of hazardous waste oil and a container of hazardous waste ink contaminated debris,
plastic buckets lacked an accumulation start date. All hazardous waste containers shall be marked with
the following information:
1. the words "Hazardous Waste"
2. name and address of generator
3. hazardous properties
4. physical state
5. composition (contents)
6. accumulation start date
Immediately label these containers and ensure that all containers are marked with all the required
information. Since it cannot be determined how long the containers of hazardous waste oil and
hazardous waste ink contaminated debris, plastic buckets have been on site, immediately contact a
licensed hazardous waste hauler to dispose of this waste under manifest and submit a copy of the
manifest to the EHD by 6-11-12.
808. A container of Universal Waste(UW) batteries were only labeled as used batteries and lacked the
label of"Universal Waste". A UW handler shall label or mark UW to identify the type of waste. Batteries
(i.e., each battery), or a container in which the batteries are contained, shall be labeled or marked clearly
with the following phrase: "Universal Waste-Batteries". Correct immediately by marking the UW batteries
with the words "Universal Waste-Batteries".
809. The facility did not demonstrate when the UW batteries on site became a UW. A UW handler shall
accumulate UW for no longer than one year from the date the UW was generated, or was received from
another UW handler. A UW handler shall be able to demonstrate the length of time that the UW has
been accumulated from the date it became a waste or was received. The UW handler may make this
demonstration by: (1) Placing the UW in a container and marking or labeling the container with the
earliest date that any UW in the container became a waste or was received. (2) Marking or labeling the
individual item of UW with the date it became a waste or was received. (3) Maintaining an inventory
system on site that identifies the date the UW being accumulated became a waste or was received. (4)
Maintaining an inventory system on site that identifies the earliest date that any UW in a group of items
of UW or group of containers of UW became a waste or was received; (5) Placing the UW in a specific
accumulation area and marking or labeling the area to identify the earliest date that any UW in the area
became a waste or was received; or (6) Any other method which clearly demonstrates the length of time
that the UW has been accumulated from the date it became a waste or was received. Correct
immediately by demonstrating when the UW lamps became a UW.
ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE.
THIS FACILITY IS SUBJECT TO REINSPECT ON AT ANY TIME AT EHD'S CURRENT HOURLY RATE.
EHD Inspector: R ve By: Titlt\` 4f
Aris Cacapit (209) 468-3453 V11 ly
EHD 23-02-003 Rev 07/12/11 CONTINUAT N FORM
