Ms. Angela Lee <br /> Huntbrook Holdings,LLC <br /> February 12, 2014 <br /> Page 5 <br /> The additional soil and groundwater assessment activities were performed between October 7 and <br /> December 3, 2013, in accordance with the field methods and procedures described in Enclosure D. <br /> The drilling and logging was performed under the supervision of a Califomia-licensed professional <br /> geologist. <br /> Pre-Intrusive Field Work Activities <br /> RDM completed the following tasks prior to performing any intrusive work at the site: <br /> • Underground Utility Location: Underground Service Alert (USA) North was notified on <br /> October 1, 2013 (USA Ticket No. 0385879) to allow USA North's member subscribers' <br /> adequate time to mark the locations of their underground utilities. In addition to notifying USA <br /> North, on October 4, 2013, RDM contracted a private utility locator, Cruz Brothers Locating of <br /> Soquel, California, to mark the locations of other potential subsurface utilities in the vicinity of <br /> • the proposed drilling locations;those not owned by the USA North member subscribers. <br /> • Site Health and Safety Plan: A site specific health and safety plan (SHSP) was prepared to <br /> identify potential site hazards and appropriate personal protective equipment (PPE), provide <br /> emergency contact information, and directions to nearest hospital or treatment facility. A copy <br /> of the SHSP was maintained on-site during fieldwork and daily tailgate meetings were <br /> conducted each morning prior to commencing with work. <br /> • Permits: Monitoring well construction permits were obtained from the San Joaquin County <br /> Environmental Health Department (SJCEHD) for the four monitoring wells (MW-4, MW-6, <br /> MW-7 and MW-8), the CPT boring (CPT-1), and the soil boring (RDM-1). Copies of the <br /> permits were maintained on-site during drilling and are included in Enclosure E. <br /> r <br /> • Concrete Coring: On October 8, 2013,Cal West Concrete Cutting of Manteca, California,was <br /> conducted concrete coring at two locations: one 4-inch diameter hole for boring RDM-1, and <br /> one 12-inch diameter hole for monitoring well MW-7. <br /> 40 Installation of Monitorinu Wells MW4,MW-6,MW-7 and MW-8 <br /> Between October 7 and October 9, 2013, RDM supervised the installation of four groundwater <br /> monitoring wells (MW-4, MW-6, MW-7 and MW-8). Monitoring well MW-8 was installed in the <br /> western portion of the subject property and the remaining three monitoring wells were installed on off- <br /> site properties to the north, east and south of the subject property. The purpose for the monitoring wells <br /> is to further assess potential petroleum hydrocarbon impacts to groundwater, groundwater flow <br /> direction, and groundwater hydraulic gradient of the water-table aquifer beneath the subject property. <br /> The locations of the monitoring wells are illustrated on Figure 2. The monitoring wells were installed <br /> by All Well Abandonment (C-57 License No. 848359) of Rancho Cordova, California, using a track- <br /> mounted, CME-75, limited access, drill rig equipped with 8-inch diameter hollow-stem augers. <br /> Monitoring wells MW-4, MW-6 and MW-7 were installed to a depth of 65 fbgs and monitoring well <br /> MW-8 was installed to a depth of 60 fbgs. The wells were drilled and constructed under the supervision <br /> of a California-licensed professional geologist. <br />
