The contingency plan did not have an emergency coordinator. The <br /> contingency plan must include: <br /> 1. Description of actions facility personnel will take in response to fires, explosions, or any sudden or non-sudden <br /> release of hazardous waste to air, soil or surface water at the facility, <br /> 2. Description of arrangements made with local police departments, fire departments, hospitals, contractors and <br /> State and local emergency response teams to coordinate emergency services; <br /> 3. List of names, addresses, and phone numbers (office/home/cell) of all persons qualified to act as emergency <br /> coordinator. Where more than one person is listed, one shall be named as the primary emergency coordinator and all <br /> others in order in which they will assume responsibilities; <br /> 4. List all emergency equipment, which includes location, physical description, and brief outline of its capabilities; <br /> 5. Evacuation routes for facility personnel where there is a possibility of evacuation; <br /> 6. Current telephone number for State Office of Emergency Services. <br /> If the facility has a Spill Prevention, Control and Countermeasures plan, or some other emergency or contingency <br /> plan, the owner or operator need only amend that plan to incorporate hazardous waste management provisions that <br /> are sufficient to comply with the requirements of this chapter. Immediately prepare or amend an existing plan to fulfill <br /> the requirements of this chapter. Submit a copy of the contingency plan to the EHD. This was corrected on site. <br /> This is a repeat violation, Class ll. <br /> 605 CCR 66262.34(f) Failed to completely label containers or tanks of hazardous waste. <br /> Containers of hazardous waste not properly labeled: <br /> -A black 35 gallon drum of sand blast media inside the shop did not have a hazardous property labeled. This was <br /> corrected on site. <br /> - Five 55 gallon drums (4 blue and 1 black) of contaminated diesel inside the bone yard were not labeled as hazardous <br /> waste. This was corrected on site. <br /> All hazardous waste containers shall be marked with the following information: <br /> -the words"Hazardous Waste" <br /> - name and address of generator <br /> - hazardous properties <br /> - physical state <br /> -composition (contents) <br /> - accumulation start date <br /> Immediately label these containers and ensure that all hazardous waste containers are marked with all the required <br /> information. This was corrected on site. <br /> This is a repeat violation, Class Il. <br /> Overall Inspection Comments: <br /> Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br /> documenting the corrective actions that have been or will be taken for each violation, and any supporting <br /> paperwork, by May 5, 2016. <br /> Page 4 of 5 <br />
