In a PHS/EHD letter dated May 9, 1997 you were informed that closure <br /> may be granted if the results of the March 6, 1997 groundwater samples were <br /> confirmed by results from an additional sampling event. The additional sample <br /> was to be split and analyzed by two independent laboratories. Although the <br /> results of the split samples were almost identical to each other they did not <br /> confirm the results of the March 6, 1997 sample. <br /> PHS/EHD requests that your consultant submit a closure document which <br /> complies with Appendix B of the Tri Regional Board Staff Recommendations for <br /> Preliminary Investigation and Evaluation of Underground Tank Sites. The closure <br /> report is to document whether complete remediation of the site has been <br /> achieved or that a "no further action" closure of a site is warranted and that some <br /> soil or ground water contaminants will remain. <br /> The minimum information required in the closure report includes: <br /> 1) Distance to production wells for municipal, domestic, agriculture, industry <br /> and other uses within 2000 feet of the site, <br /> 2) Site maps, to scale, of area impacted showing locations of former and <br /> existing tank systems, elevation contours, gradients, and nearby surface <br /> waters, buildings, streets, and subsurface utilities, <br /> 3) High and low groundwater levels (below ground surface), <br /> 4) Tabulated results of all sampling and analysis, <br /> 5) Vertical and lateral concentration contours of contaminants found initially, <br /> and those remaining in soil and ground water, both on and off site, <br /> 6) Mass balance calculation of the substance treated versus that remaining, <br /> A Division of San Joaquin County Health Care Services <br />
