Environmental and Engineering <br /> Group, for the above referenced site. <br /> In the work plan, WHF proposes the destruction of the abandoned drinking water well, and installation of a <br /> groundwater pumping test system in MW-3, in preparation for construction of a pump and treat system as a site <br /> remediation process. <br /> You have been aware of the requirement to destroy the abandoned well since March 17, 1992, when you were <br /> notified verbally during a meeting with PHS/EHD staff. In the PHS/EHD letter of July 14, I995, you were issued <br /> a compliance date of September 21, 1995, to destroy the abandoned drinking water well. On September 15, 1995, <br /> WHF submitted a completed well destruction permit application to this agency. On September 20, 1995, PHS/EHD <br /> issued well destruction permit 1#007350, for the destruction of the abandoned drinking water well at this site. <br /> San Joaquin County Development Title Section 9-1115.5 (e) states "Upon determination that any well is abandoned <br /> as provided in the rules and regulations adopted pursuant to this Chapter, that well must be destroyed in the manner <br /> prescribed so that entrance of degraded or contaminated water into usable aquifers, or creation of a safety hazard, <br /> are minimized." <br /> San Joaquin County Development Title Section 9-1115.5 (f) states "The Director of the Environmental Health <br /> Division is authorized after reasonable efforts to eliminate pollution, contamination, or a safety hazard, to enforce <br /> the permanent abandonment by destruction of any well that is polluted, contaminated, or is located as to become <br /> polluted or contaminated or is a safety hazard. The Director is authorized to destroy any such well and to recover <br /> the cost of the destruction from the owner of the property on which the well is located." <br /> PHS/EHD requires that this abandoned well be destroyed by November 30, 1995, in accordance with San Joaquin <br /> County Well Standards. Failure to comply, may result in further legal action to be taken against you. <br /> Prior to installation of any form of treatment at this site a Corrective Action Plan shall be submitted to this <br /> department for approval and concurrence. As stated in Article 11,Chapter 16, Division 3, Title 23 of the California <br /> Code of Regulations (the Corrective Action Requirements) a Corrective Action Plan (CAP) shall be submitted to <br /> the regulatory agency for review and concurrence when the soil and water investigation phase is complete. The <br /> Corrective Action Plan shall Include an assessment of the distribution of the contamination present at the site, a <br /> feasibility study to evaluate alternatives for mitigating the actual or potential adverse effects of the unauthorized <br /> release, and propose at least two alternatives to achieve cleanup levels for waters with current or potential beneficial <br /> uses for which numerical objectives have been designated. <br /> A Division of San Joaquin County Health Care Services <br />
