Assuring that personnel are aware of the potential hazards associated with site <br /> operations. <br /> 4. Monitoring the safety performance of all personnel to ensure that the required <br /> work practices are employed. <br /> 5. Correcting any work practices or conditions that may result in injury or exposure <br /> to hazardous substances. <br /> 6. Preparing any accident/incident reports (see attached Accident Report Form). <br /> 1.4 Project Employee <br /> Project Employees involved in on-site investigations are responsible for: <br /> 1. Taking all reasonable precautions to prevent injury to themselves and to their <br /> fellow employees. <br /> 2. Implementing Health and Safety Program and reporting to the PSO for any action <br /> deviating from the anticipated conditions described in the Health and Safety Plan. <br /> 1.5 Special Instructions <br /> It is the responsibility of the Project Manager and the Health and Safety Manager to <br /> review the available site information to determine, to what degree the Health and Safety <br /> Plan will be implemented. Each site will be handled on a case by case basis. This site, <br /> because of the nature and known levels of contaminates, has been identified as a Level "D" <br /> entry. Based on U.S. EPA rating criteria for entry, this site requires a minimum of <br /> protection. This will include rubber boots, tyvek, gloves, and hard hats. <br /> Should for any reason, conditions change, the site will be elevated to an appropriate level <br /> �. of entry for safe working, based on the Project Safety Officer (PSO) and Project Manager <br /> (PM)- <br /> Should at any time the level of entry change, the remaining specific safety equipment and <br /> precautions can be implemented by the manager as required.
