Minimum 3 soil <br /> samples per boring will be collected for laboratory analyses of diesel and BTEX. <br /> 2. The soil borings will be converted to monitoring wells. Perform quarterly sampling of the monitor <br /> wells. <br /> 3. Conductor casing will be used before installation of the monitoring well if deeper aquifer is to be <br /> investigated. <br /> 4. Perform a 2000-foot receptor well survey. <br /> COMMENTS AND CONDITIONS: <br /> 1. Collect and analyze groundwater samples from the domestic water well south of the former UST. <br /> Collect water sample from the well head and allow at least 15 minutes of flow before sampling in <br /> accordance to California Code of Regulations,Title 22,Division 4,Chapter 15,Article 5.5,Section <br /> 64445. <br /> 2. Section 2.2 of the work plan called for placing bentonite or cement and bentonite grout at the bottom <br /> of the boring as necessary before installing the monitoring well casing. If cement is used,at least 24 <br /> hours must be allowed for of set up before placing well casing and sand pack. <br /> 3. Obtain well permit and submit appropriate permit fee to PHS/EHD before commencement of work- <br /> Permits <br /> orkPermits are issued to G57 licensed holders only. <br /> 4. Selection of samples for analyses must include PHS/EHD comments and agreement. <br /> 5. Schedule work with PHS/EHD before finalizing the work schedule. <br /> 6.
