PHS/EHD has the following comments for <br /> Sampling had been scheduled by Gary Mosted of Delta Environmental to occur at 2:00 p.m., August 15, <br /> 1991. Mary Meays and Nick McKibbin of PHS/EHD arrived at 2:05 P.M. to observe sampling. A site <br /> inspection was made by Mary Meays prior to the arrival of Gary Mosted. Mr. Mosted measured the depth <br /> to groundwater in each well. Due to the lateness of the hour, PHS/EHD would not have been able to stay <br /> throughout the well purging to observe sampling. PHS/EHD was unable to verify that the proper equipment <br /> was present to measure sampling parameters and to maintain adequate cold storage for sample storage and <br /> transport. It was evident that good quality control/quality assurance (QC/QA) practices could not be <br /> achieved. A Notice to Abate was issued to Mr.Mosted requesting that sampling be rescheduled. A copy is <br /> enclosed for your information. <br /> Lisa Rainger, Delta Environment Project Manager, was notified August 15, 1991, of the Notice to Abate <br /> issuance. PHS/EHD was assured that QC/QA practices would be strictly adhered to in the future. <br /> Monitoring wells were sampled August 20, 1991, under inspection of PHS/EHD. The grouts surrounding <br /> the traffic boxes protecting MW2 and MW3 were noted to be severely damaged. Repair monitoring wells <br /> surface seals within 14 days from the date of this letter.
