`..� NOW <br /> Ground Zero Analysis, Inc. <br /> SITE SAFETY PLAN <br /> Other Hazards (Check if applicable) <br /> x Noise Activities likely to generate noise exceeding 85 dB: <br /> Wear hearing protection during these activities. <br /> Heat Stress Symptoms: <br /> ✓ Heat Cramps: Muscular pains and spasms. <br /> ✓ Heat Exhaustion: Cool, pale, moist skin; heavy sweating; dilated <br /> pupils, headache, nausea, dizziness, vomiting, <br /> near normal body temperature. <br /> ✓ Heat Stroke: Hot, red skin; very small pupils; high body <br /> temperature; reduced sweating. <br /> Mitigation: Cool place for breaks: In Facility Building <br /> Monitor personnel using pulse rate and/or blood pressure, body temperature take frequent <br /> breaks in shaded area. Unzip or remove PPE during breaks and results of personnel monitoring. <br /> Provide drinking water and/or electrolyte replacement drink. Record the time and duration of all <br /> breaks and results of personnel monitoring. Heat stroke victims must receive emergency <br /> medical care. <br /> ❑ Hypothermia/Frostbite Symptoms: <br /> ✓ Hypothermia: Shivering, apathy, loss of consciousness, <br /> decreasing pulse and breathing rate. <br /> ✓ Frostbite: White, then grayish yellow progressing to <br /> grayish blue skin, cold, numb part. <br /> Mitigation: Wear multilayer cold weather outfits covered by a wind resistant fabric. Take <br /> frequent breaks in a warm, sheltered area. Provide warm non-alcoholic drinks. For frostbite <br /> victims warm injured part gradually, do not rub! Warm hypothermia victims and transport to <br /> emergency medical care. <br /> x Underground/Overhead Utilities (show location of utilities on site map) <br /> of How were underground utilities identified?
