Manthey Road, Lathrop
Page 2
Please be advised that the above referenced property has been placed on the
EHD list of UIC sites and will remain an open case until the proper investigation
has been conducted. You should obtain the services of an environmental
consulting firm (list enclosed) to prepare a work plan to investigate the septic
system and if necessary the area of the abandoned well immediately.
Enclosed is a master file record form that is to be completed and submitted with
a fee of $267 that covers the first three hours of our work on the site ($89/hour).
If more than three hours is spent on oversight of the site investigation, you will be
sent a bill calculated by multiplying the $89/hour fee times the number of
additional hours. The master file record form and $267 should be submitted with
the work plan from an environmental consultant for further site investigation.
If you have any questions contact me at (209) 468-3449.
Donna Heran, REHS, Director
Environmental Health Department
—M' A-5
Margaret Lagorio, Supervising REHS
Unit IV
Enc.
