20 October 2014 AGE Project No. 09-1729 Page 3 of 3 depth with a thick grout mixture (no less than 6 gallons of water per 94 bag of cement); all water forced out during the initial placement of the grout will be containerized and properly disposed. Once the well is filled with the grout mixture, the well head will be fitted with a pressure tight adapter and air pressure will be added at approximately 25 pounds per square inch (psi) of pressure for a minimum of 10 minutes. The added pressure will be used to force a significant portion of the grout mixture out through the screen, sealing the well. Once pressurization is completed, a hole at least one-foot in diameter greater than the original diameter of the borehole will be excavated to a depth no less than three and one-half feet bsg, exposing the well casing. The well casing will then be cut and removed six-inches above the excavation and grout will be used to fill the boring ("mushroom cap") to near surface grade. The remaining borehole will be finished at the surface with a concrete mixture. Rinseate generated during drilling and decontamination activities will be containerized in properly labeled Department of Transportation (D.O.T.)-approved 55-gallon drums and soil will be contained in an onsite soil bin and both will be stored on-site until proper disposal is arranged to a suitable non-hazardous waste facility. Upon completion of field work and disposal waste, a report documenting the field activities and procedures, including documentation of waste disposal, will be prepared and submitted to the SJCEHD.
