20 October 2014 <br /> AGE Project No. 09-1729 <br /> Page 3 of 3 <br /> depth with a thick grout mixture (no less than 6 gallons of water per 94 bag of cement); <br /> all water forced out during the initial placement of the grout will be containerized and <br /> properly disposed. Once the well is filled with the grout mixture, the well head will be <br /> fitted with a pressure tight adapter and air pressure will be added at approximately <br /> 25 pounds per square inch (psi) of pressure for a minimum of 10 minutes. The added <br /> pressure will be used to force a significant portion of the grout mixture out through the <br /> screen, sealing the well. Once pressurization is completed, a hole at least one-foot in <br /> diameter greater than the original diameter of the borehole will be excavated to a depth <br /> no less than three and one-half feet bsg, exposing the well casing. The well casing will <br /> then be cut and removed six-inches above the excavation and grout will be used to fill <br /> the boring ("mushroom cap") to near surface grade. The remaining borehole will be <br /> finished at the surface with a concrete mixture. <br /> Rinseate generated during drilling and decontamination activities will be containerized in <br /> properly labeled Department of Transportation (D.O.T.)-approved 55-gallon drums and <br /> soil will be contained in an onsite soil bin and both will be stored on-site until proper <br /> disposal is arranged to a suitable non-hazardous waste facility.
