California Stop Page 3 of 3 <br /> 2224 Manthey Road September 26, 2014 <br /> Stockton, California 95206 <br /> All groundwater displaced to the surface during grout installation shall be pumped out of <br /> the excavation to allow for EHD inspection and verification that the well has been <br /> properly destroyed. Contaminated groundwater pumped out of the excavation shall be <br /> containerized for proper disposal. <br /> After the EHD has approved the well destruction scope of work proposal and the public <br /> participation comments have been properly addressed, you may submit an EHD Well and <br /> Boring Permit Application form along with a Licensed Contractors' Declaration (LCD), Master <br /> File Record (MFR) form, and appropriate fees for each parcel. The permit fee for the <br /> destruction of each well is $130, along with a $390 Inspection Activity fee per parcel. A review <br /> of the site records indicate that eleven monitoring wells, four soil vapor extraction wells, and two <br /> ozone injection wells currently are installed at this site. A fee of $2,600.00 (17 x $130 + $390) <br /> will be required for the well destruction permit. <br /> The EHD will issue a closure letter once the sixty-day public participation notification <br /> requirements and opportunity for public participation in the case closure decision process have <br /> been completed in accordance with the LTCP, certification that all record owners of fee title <br /> have been notified of case closure consideration has been received by the EHD, all wells have <br /> been property destroyed and waste manifests have been submitted to this agency. <br /> Please contact Vicki McCartney at (209) 468-9852 or by email at vmccartney(@,,slcehd.com if <br /> you have any questions concerning this letter. <br /> Sincerely, <br /> Victoria L.
