PUBLIC NOTICE <br /> Fact Sheet <br /> Pending Case Closure for Leaking Underground Storage Tank Site <br /> (Name of the consulting firm), on behalf of the San Joaquin County Environmental Health Department <br /> (EHD), invites public comment regarding consideration by the EHD to grant regulatory closure for the <br /> leaking underground storage tank (LUST) case at the subject site (Site) identified below: <br /> SUBJECT SITE: <br /> Site Name: (site name) GeoTracker Global ID No. (T#) <br /> Site Address: (site address) SJCEHD Site Code: (Code #) <br /> PUBLIC COMMENT PERIOD: 60 Days — from (date) to (date) (note: the 60-day time frame should <br /> commence on the anticipated mailing and posting date of the EHD-approved Public Notice) <br /> CASE SUMMARY: <br /> The SJCEHD and the Central Valley Regional Water Quality Control Board (RWQCB), in cooperation <br /> with current property owner name (property owner), the responsible party name who may be same as <br /> owner. (responsible party) and name of consulting firm (environmental consultant) have evaluated soil <br /> and groundwater conditions, and potential health concerns at the subject Site resulting from <br /> unauthorized release of (gasoline, diesel, etc.) from an underground storage tank (UST) system. The <br /> unauthorized release from the UST system was discovered (date month/year) when the tanks <br /> dispensers, piping or system) was/were (tested, upgraded replaced or removed). At the current time <br /> there (is/is not) an active UST system on the Site. <br /> Soil and groundwater investigations were conducted from month/year to month/year, to characterize <br /> impacted soil and/or groundwater. Consultinq firm determined the extent to which the petroleum <br /> hydrocarbons have significantly impacted soil and groundwater. Note whether or not the extent of <br /> contamination remained on-site or extended off-site (how far). Type of remediation and month/year <br /> was conducted to remediate the soil and groundwater at this site. (Consulting firm) estimates that <br /> approximately xxxx pounds of contaminants were removed from the site through active remediation. <br /> Potential negative effects of the residual contamination on human health and the environment were <br /> evaluated by (the consulting firm), and found to be negligible. On the basis of groundwater monitoring <br /> and the site characterizations, and the recommendation of (the consulting firm) the SJCEHD has <br /> concluded that the Site status is protective of the environment and human health for current and future <br /> Site use and for adjacent off-site areas. <br /> The EHD will submit Case Closure Summary to the California Region Water Quality Control Board <br /> (CRWQCB). Once the EHD receives and responds to the comment from CRWQCB and has <br /> addressed all public comments received, the EHD anticipates issuing a No Further Action Required <br /> letter for the subject Site. <br /> WHERE TO GET MORE INFORMATION <br /> Site information can be obtained from the GeoTracker website at http://geotracker.waterboards.ca.gov/. <br /> Upon written request, the Site case file is available for review at the EHD office located at 1868 East <br /> Hazelton Avenue, Stockton, California 95205. An EHD file review request form can be obtained from <br /> the SJCEHD website at http://www.sjcehd.com. <br /> HOW TO PARTICIPATE <br /> Interested individuals have the opportunity to learn more about this case and voice concerns regarding <br /> its pending closure by submitting comments to (case worker) at the SJCEHD.
