page 3, 4075 Main St. <br /> In addition to the work plan, all information missing from previously submitted reports <br /> must be forwarded to PHS/EHD by April 23, 2001 as well. Specific information missing <br /> from the reports includes the following. <br /> 1. A comprehensive and detailed site map. <br /> Site maps must be to scale and must show the location of the former <br /> underground storage tanks, the current underground tanks, buried pipes and <br /> utilities, septic systems and leachfields, all above ground structures including <br /> utility lines as well as nearby landmarks such as rivers and potential sensitive <br /> receptors. <br /> 2. A geologic description of the site. <br /> A complete geologic description of the site subsurface lithology must be included <br /> and updated in every site characterization report. Complete, accurate and <br /> properly labeled site cross-sections should also be included. A standard <br /> explanation must be included with the cross-sections that defines all units, <br /> abbreviations, and symbols used, as well as the horizontal and vertical scales <br /> used to create the drawing. <br /> 3. Comprehensive soil and groundwater monitoring analytical data must be <br /> presented in tables. <br /> The tables must show every contaminant analyzed with the date of each <br /> sampling event listed chronologically. Data tables must be updated in each <br /> report to show trends of contaminant concentrations over time. In addition, it is <br /> standard industry protocol to report groundwater analytical results in parts per <br /> billion, or micrograms per liter, not parts per million. <br /> Refer to the Tri-Regional Board Staff Recommendations for Preliminary Investigation <br /> and Evaluation of UndergroundTank Sites (Appendix A)for required reporting <br /> specifications.
