El Dorado Street, Room 317 <br /> Stockton, California 95202 <br /> Subject: Paved Over Groundwater Monitoring Well <br /> Beacon Station No. 474 <br /> 3440 East Main Street, Stockton <br /> Mr. Enders: <br /> At the request of Ultramar Inc. (Ultramar), Horizon Environmental Inc. (Horizon) is <br /> performing environmental work as described in the Work Plan for a Remedial Investigation <br /> (Horizon, June 1, 1998) and the Letter Addendum to Work Plan for a Remedial Investi ation <br /> (Horizon, December 28, 1998) at the above-referenced site. These work plans were <br /> submitted to the San Joaquin County Public Health Services - Environmental Health <br /> Division (SJCPHS-EHD) and subsequently approved by the SJCPHS-EHD. As approved by <br /> the SJCPHS-EHD, one of the ongoing tasks to be performed at the site is to measure <br /> groundwater levels and perform groundwater sampling of ALL existing monitoring wells <br /> associated with the site. However, since November 1997 groundwater monitoring well MW-4 <br /> has not been accessible for monitoring and sampling because repaving of East Main Street has <br /> covered the wellhead box and rendered the well inaccessible. <br /> On February 20, 1996, Horizon received Revocable Permit No. 96-04 from the City of <br /> Stockton Department of Public Works (CSDPW) for the installation of two offsite <br /> groundwater monitoring wells within City streets. On May 9, 1996, groundwater <br /> monitoring well MW-4 was installed along the northern side of the East Main Street right- <br /> of-way. As documented in Item #2 of Revocable Permit No. 96-04, "the precise location <br /> shall be determined in the field by the City of Stockton". Since this well must be accessed <br /> for future groundwater monitoring episodes, Horizon is requesting that this well be located <br /> and uncovered by the City of Stockton so that groundwater monitoring of this well can <br /> continue in the future. The locations of the previously installed groundwater monitoring <br /> wells (including well MW-4) are shown on the attached Surveyor's Map (Ron Archer Civil <br /> Engineering, June 10, 1996). <br /> Additionally, in October 1999, attempts were made to install offsite groundwater monitoring <br /> well MW-10 along the southern side of the East Main Street right-of-way, as presented in <br /> Revocable Permit No. 99-13.
