Comments: <br /> Routine inspection conducted on this date and the following was noted: <br /> 1.)Violation T14 CCR§ 18460.2 -A CTL search done for this hauler from 3/16/15 to 8/4/16 resulted in 276 <br /> manifests. 46 of the 276 manifests were not filled out correctly. CTLs were missing dates, did not have load <br /> types checked, and had load amounts written incorrectly. Facility has not been manifesting the delivery tires <br /> back to this location from clean up sites on days when disposal locations are not able to take the waste tires. <br /> A. A waste tire manifest shall be completed for each pick-up or delivery of any waste or used tires. Waste tire <br /> manifests shall be filled out completely and accurately. <br /> B. Ensure subsequent manifests are filled out completely and accurately. If the waste tires are hauled back to <br /> this facility to be held before being delivered to a disposal facility on a different day, then this is considered a <br /> delivery. A manifest must then be filled out for the delivery of the waste tires to this location. On the day that the <br /> waste tires will be hauled to the disposal facility, a pick-up manifest must be filled out before hauling the tires to <br /> that location. <br /> 2.) A/C T14 CCR§ 18459.2.1 -The following CTLs for this hauler did not appear in the WTMS search: 5732956 <br /> (A,B), 5732960-A, 5732953 (A,B,C), 5732952 (A,B,C), 5732957-A, 5732958 (A,B), 5732959 (A,B), 5732955 <br /> (A,B,C), 5732954 (A,B,C). <br /> A. CTL forms shall be submitted to CalRecycle within 90 days of the load shipment. <br /> B. Ensure CTL forms are submitted to CalRecycle within 90 days. <br /> Notes: <br /> 1.)TPID certificate is posted in the office. <br /> 2.) Permission to inspect was granted by Kristina Contreras (Staff Secretary). <br /> 3.) Waste tires were counted directly and are stored inside the facility. <br /> 4.) Retain all waste tires manifests, disposal receipts, and logs for 3 years. <br /> 5.) Facility only stores it's waste tires at this location. Waste tires picked up at cleanup sites are generally <br /> hauled directly to a disposal facility unless that facility is closed or cannot accept the tires. In those cases the <br /> tires are hauled back to this location and hauled to the disposal facility on a different day. <br /> *Re-inspection will occur on or after November 16, 2016.All violations must be corrected by this date. <br />
