Original permit through this office covered piping removal and <br /> replacement only. PHS/EHD (Mike Kith) witnessed sampling 7-27-95. Excavation and removal <br /> of suspected petroleum hydrocarbon contaminated soil has occurred without submitting a <br /> workplan to PHS/EHD also not witnessed by PHS/EHD staff. No reports documenting suspected <br /> releases have been received by this office (Prop. 65/Unauthorized Release etc.) prior to a verbal <br /> request by PHS/EHD today 8-10-95.
