Page I of I
r
Mike Infurna [EH]
From: Mike Infurna [EH]
Sent: Wednesday,September 14,2011: 1:09 PM
4 To: 'amber.nickens@s.tahtec.com'
° Subiect: 76 Station#5886
` Amber,
s
we spoke on the phone today about some permit application concerns for destroying the wells at
this site. Initially, I told you I needed another application and $1500 fee.
after reviewing the site map, I realized you PAID for all the 27 wells in one check, but mistaking
included onsite and offsite wells on ONE application. I then left you a voice mail message f
3. stating I could make the one application work by duplicating it and correcting the land owner
information to reflect "City of Stockton" as the correct landowner for the 9 offsite wells.,
i
but I neglected to advise that each application requires an inspection fee of$375. then to.each
application, a 'per well' charge of$125 is added.
3
i there are 18 wells ONSITE. doing the math, 18 x $125= $2,250. add the inspection fee of
$375 and the ONSITE permit application is $2,625.
i
i
and there are 9 wells offsite: 9 x$125 -$1125 and add the $375 inspection fee, the OFFSITE
I permit total due is $1;500. I
you submitted check.#297869 for $3,750. II
E
application fees needed total $4,1.25. leaves a balance you owe us at $375. and not
the$1500 1-initially advised or the NO money due like I said in my phone call.
please forward $375,for the offsite permit as soon as you:can. you may or may not submit a
y revised'permit application for the OFFSITE wells;,your choice. just let me know you got this. i
j 1 am going to start processing the ONSITE permit since I have all l need for it and I have time
today to do it, a
Since you only need to send money, I recommend you start setting up dates with the driller to do
this work. I am HAMMERED with, well destructions (475 over the next four months) and finding
office time is a premium. This 76 site.is a HIGH priority with the EHDand the SWRCB and 1
have every intention of getting this site OFF my desk and a NFA letter on yours before the end of
the year.
i
Please do whatever you can to expedite all we need to do to meet this 2011 deadline.
thank you.,
Mike Infurna E
9/'14/2011 1
