March Lane LOP Case#: 1176 <br /> Stockton, CA 95215 Global ID#: T0607700718 <br /> The San Joaquin County Environmental Health Department(EHD) has reviewed Work Plan to Destroy <br /> Existing Monitoring and Remedial Wells (Plan) dated April 18, 2011 and an electronic mail plan <br /> revision dated May 9, 2011. The revision was submitted after San Joaquin County Well Standard <br /> requirements were discussed by phone between the EHD and Mr. Mark Bare of ATC consulting. <br /> The Plan was required by the EHD in correspondence dated March 3, 2011 to address the proper <br /> destruction of the monitoring and remediation wells installed for this site's required <br /> investigation/cleanup. Originally the Plan called for pressure grouting the air sparge wells (AS-1 <br /> through AS-10). The EHD advised Mr. Bare that in order to allow the pressure grout destruction of the <br /> wells, laboratory data for either soil or groundwater analyses must be provided to prove that <br /> contamination is not present. No data was presented or available to supportthat the wells are not in a <br /> contaminated area so the remediation wells must be destroyed by drilling out/removing the contents of <br /> the wells. <br /> On May 9, 2011, Mr. Bare submitted an electronic mail proposal modifying the Plan. The proposal <br /> substituted 'over-drill'destruction procedures for the pressure grout procedures included in the Plan for <br /> the AS wells. The EHD accepts the revision as adequate and necessary and approves the proposal. <br /> [H&S 25296.10(c)(3)]. <br /> Please submit a Well Permit Application form, a $2562 fee, and a completed and signed Master File <br /> Record Form for the on-site well destructions. The fee reflects the application fee of $366 plus the <br /> $122 fee per well for each of the 6 nested, 11 single, and 1 UST well (18 total) present on-site. <br /> Another EHD Permit Application Form and Master File Record (MFR) form is required for the off-site <br /> well destructions. The EHD will waive the$366 permit application fee for the off-site permit application <br /> since the work will be performed during the on-site well destruction phase.
