5. Entrance and exit locations must be designated, and emergency escape routes delineated. Warning <br /> signals for Site evacuation must be established by the SSHO before field activities. <br /> 6. Communications using radios,hand signals, or other means,must be maintained between initial <br /> entry members at all times. Emergency communications should be prearranged in case of radio <br /> failure,the necessity for evacuating the Site, or other reasons. <br /> 7. Wind indicators visible to all personnel should be strategically located throughout the Site. <br /> 8. Personnel and equipment in the contaminated area should be minimized, consistent with effective <br /> Site operations. <br /> 9. Work areas for various operational activities will be established. <br /> 10. Procedures for leaving a contaminated area will be planned and implemented before going to the <br /> Site. Work areas and decontamination procedures will be established based on expected Site <br /> conditions. <br /> 11. Frequent and regular inspections of Site operations will be conducted by the SSHO to check <br /> compliance with this HASP. If changes in operation occur,the HASP must be modified to reflect <br /> these changes. <br /> 12. All electrical equipment(power tools,extension cords, instruments,radios, etc.)will conform with <br /> ATC Policy No. 12 (Electrical) The SSHO will ensure that electrical equipment is free from <br /> recognized hazards that may cause physical harm to employees. <br /> 13. Fire prevention and protection(appropriate signs for flammable liquids, smoking areas, storage <br /> areas of combustible or flammable materials, etc.)will be according to ATC Policy No. 18,Fire <br /> Protection. <br /> 14. Site Tailgate Safety Meetings will be held daily to discuss anticipated Site conditions and daily <br /> activities. This meeting will be summarized in field logbooks and the Tailgate Safety Meeting <br /> Form(see Appendix Q. <br /> HASP Level III <br /> Rev.—(September 17,2012) <br /> 8-2 <br />
